Disclosure: Zynix AI makes one of the platforms in this guide. Every description of another vendor comes from that vendor’s own website as of November 7, 2026, linked in the source notes. This is not a ranking: products change, so confirm current capabilities with each vendor.

Key takeaways

An AI patient engagement platform reaches patients by voice, text or digital channels to get them to the care they are due for: a screening, an annual wellness visit, a follow-up after discharge, a refill. For value-based care organizations, the test is whether the outreach closes gaps and gets visits booked, not how many messages go out.

  • Voice-agent platforms include Hippocratic AI, Innovaccer’s Gravity Voice Agents, Hyro and Infinitus; Notable, Luma Health, Artera and Commure Engage combine agents with scheduling and messaging; Lirio and CareMessage focus on personalized or text-first outreach.
  • Most vendors sell software; Artera pairs its platform with dedicated build teams, and CareMessage is a non-profit focused on safety-net providers.
  • Under a 2024 FCC ruling, calls that use AI-generated voices fall under the TCPA’s artificial-voice rules, so consent and disclosure belong in every evaluation.
  • Zynix AI’s voice and text agents run outreach from the care plan and risk signals in the same platform, book the visit and route clinical questions to your team by rule.

What does an AI patient engagement platform do?

A Shared Savings Program ACO “agrees to be held accountable for the quality, cost, and experience of care” of its assigned Medicare population, according to CMS. Much of that accountability depends on patients doing something between visits: booking the wellness visit, completing the screening, keeping the follow-up after a hospital stay, refilling the medication. Patient engagement platforms do the reaching out at a scale care teams cannot staff by phone.

The platforms in this guide combine some of five jobs:

  • Outbound outreach: campaigns to patients with open care gaps, due wellness visits or missed refills.
  • Scheduling: booking, reminders and rescheduling, ideally in the same conversation.
  • Inbound access: answering calls and messages, including after hours, and routing them.
  • Care-journey follow-up: check-ins after discharge or a procedure, and between chronic care visits.
  • Escalation: handing clinical questions to a nurse or clinician under rules the organization sets.

AI voice calls and consent

In February 2024 the FCC confirmed that the TCPA’s restrictions on “artificial or prerecorded voice” calls “encompass current AI technologies that generate human voices.” Such calls “require the prior express consent of the called party to initiate such calls absent an emergency purpose or exemption,” and the messages must identify the entity responsible for the call. Ask every vendor how it records consent, identifies the caller and honors opt-outs, and review the exemptions that apply to your program with counsel.

The vendors, one by one

Hippocratic AI

Hippocratic AI builds “clinical, non-diagnostic voice AI for patient-facing clinical conversations,” and states that its “AI agents do not diagnose or prescribe.” Its payer offerings include AI Star Rating Improvement, with “Super HEDIS multi-gap campaigns with omni-topic call handling,” and AI Chronic Care Management, with check-ins for conditions such as diabetes, heart failure and COPD. On the provider side, its readmission offering runs “transition-of-care calls in the 24-72-hour window with follow-up appointment capture and in-call booking to support TCM billing.” Acute symptoms are escalated to registered nurses using Schmitt-Thompson protocols. It serves health plans and risk-bearing organizations, health systems and medical groups, and life sciences companies.

Notable

Notable describes itself as “an agentic AI platform for healthcare that automates administrative workflows across patient access, care operations, and revenue cycle,” with the EHR remaining the system of record. For value-based care, its “AI agents identify and engage patients to schedule screenings and close care gaps,” and the same flow combines chart review, multi-channel outreach and self-scheduling, with results recorded in the EHR. It names bi-directional integrations with Epic, Oracle Health, MEDITECH Expanse and athenahealth, and positions itself for health systems. See Zynix AI vs Notable for a closer comparison.

Innovaccer Gravity Voice Agents

Innovaccer’s Gravity Voice Agents “talk with patients and payers by phone, on both inbound calls patients place and outbound calls the agents make,” and the platform also reaches patients by SMS and chat. Named uses include reaching “patients overdue for screenings,” detecting “refill lapses on the adherence measures,” booking the TCM follow-up visit inside the seven-day window, and scheduled check-ins for heart failure, COPD, diabetes and CKD. “When a call needs clinical or financial judgment, the agent escalates to your team with the context already gathered.” It fits best where Innovaccer already holds the patient data.

Luma Health

Luma Health, co-founded in 2015, offers a patient access platform whose Navigator handles “inbound and outbound AI conversations, via voice, text, and chat.” For care gaps, patients in a population “get outreach personalized to them, via SMS/RCS or outbound AI call,” and Luma “can remind the patient a year after their last Medicare Annual Wellness visit.” “All clinical advice and requests to speak to staff get escalated.” It names integrations with MEDITECH, Oracle Health, eClinicalWorks, Epic, athenahealth and ModMed. Luma Health also acquired Tonic Health from R1.

Artera

Artera, formerly WELL Health and founded in 2015, pairs an agentic platform with AI services: its Harmony platform plus AI Service Squads, “dedicated groups of Artera team members” who build and support workflows with the customer. Its agents cover scheduling, intake, referrals, care gap closure and post-discharge follow-up, and Artera states that they “automate administrative workflows and do not provide medical advice, diagnosis, or treatment.” It serves specialty groups, FQHCs, large IDNs and federal agencies.

Hyro

Hyro, founded in 2018, sells conversational AI agents under its Care OnCue brand for inbound and outbound patient communication across phone, chat and SMS. Its outbound agents send reminders and follow-ups for “appointments, referrals, prescriptions, coverage, and overdue care,” and it lists post-discharge follow-up among its uses. It names integrations with Epic, Cerner and athenahealth. Most of its focus is health system access centers; for payers, it handles routine member-service interactions.

Commure Engage (formerly Memora Health)

Commure announced its acquisition of Memora Health on December 20, 2024, and Memora’s site now points to Commure Engage. Engage “powers patient access and engagement across the care journey” by combining “AI call center agents, automated outreach, and deep EHR integration.” It is SMS-first, with no app or portal login, and names post-discharge recovery and symptom monitoring, chronic disease management programs and remote monitoring devices. It is positioned for health systems. See Zynix AI vs Commure.

Lirio

Lirio, based in Knoxville, Tennessee, takes a different approach: it applies behavioral science to choose the message, channel and timing for each person, and delivers “across email, SMS, push, chatbot, and more” through channels the customer already runs. For health plans, it targets gap closure programs for “measurable HEDIS and Stars performance,” along with follow-up after transitions, medication adherence and preventive visits. It serves health systems, health plans, life sciences organizations and retail pharmacies.

CareMessage

CareMessage is a technology non-profit focused on low-income populations. Its text-first platform supports outreach for “HEDIS, UDS, GPRA, and more,” lets health centers “import attributed patient lists from your health plans,” and runs gaps-in-care journeys such as diabetes check-ups through its Wellness Copilot. It also supports SDoH screening and resource outreach. It is built for FQHCs, free and charitable clinics, Tribal Health Organizations and primary care associations.

Infinitus

Infinitus offers a voice-agent platform for payers, providers and pharma. For payers, it lists welcome calls, health risk assessments, benefits questions, care gap outreach and refill reminders among the workflows its agents run, plus SDoH data collection. For providers, “Infinitus AI agents handle patient outreach, appointment coordination, and engagement workflows.” Customers can build agents themselves in its Studio or have Infinitus build them.

Zynix AI

Zynix AI’s AI Patient Engagement agents reach patients by voice calls and two-way texts in the patient’s language, at the hours your team sets. They book the visit in the same conversation, handle reminders and no-show recovery, answer after-hours calls, and route symptom questions to the on-call clinician by rule. Agents never diagnose, triage clinically or recommend treatment, and in post-discharge TCM the interactive contact is made by clinical staff while the agent handles the reminders and booking around it.

The difference from most tools in this guide is where the outreach list comes from. In the Zynix platform, outreach starts from the same patient record that drives Predictive Risk and the care plan, so a rising-risk flag or an open gap becomes a call with an owner and a deadline. Zynix connects to 30+ EHR systems across 300+ connected instances. Analytics tells you who. Zynix makes sure something happens.

How to choose an AI patient engagement platform

  • You need care gaps and wellness visits closed across many practices: ask how the platform gets its outreach list, how it books into each practice’s schedule, and how a closed gap reaches your quality reporting.
  • Your main problem is call volume and access: compare inbound-focused platforms such as Hyro, Luma Health and Notable.
  • You are a health plan running HEDIS and Stars campaigns: compare Hippocratic AI, Lirio and Infinitus, and ask how member consent and plan branding are handled.
  • You serve safety-net patients: look at CareMessage’s text-first model and multilingual support from any vendor.
  • You want outreach tied to risk and care plans: check whether the engagement tool shares a record with your risk model and care management, which is where Zynix AI fits.

For every vendor, ask whose clinicians take escalated calls, what the agent is not allowed to discuss, how consent and opt-outs are recorded, and which results it can show for your own population. For a closer look at the trade-offs, see AI voice and text agents vs a call center.

Frequently asked questions

What is the best AI patient engagement platform for value-based care?

There is no single best platform; the right one depends on what your program needs patients to do. Health plans running HEDIS and Stars campaigns often compare Hippocratic AI, Lirio and Infinitus. Health systems focused on access compare Notable, Luma Health, Hyro and Artera. Safety-net providers look at CareMessage. ACOs and MSOs that want outreach driven by risk and care plans look at Zynix AI.

What is the difference between patient engagement software and an AI voice agent?

Patient engagement software traditionally sends reminders, texts and campaigns. AI voice agents hold a two-way phone conversation: they can answer questions, book the visit during the call and hand anything clinical to a person. Several platforms in this guide now offer both.

Do AI patient outreach calls need patient consent?

In February 2024 the FCC confirmed that calls using AI-generated voices count as artificial-voice calls under the TCPA, which require the called party’s prior express consent absent an emergency purpose or exemption, and require the caller to identify itself. Review consent and the exemptions that apply to your program with counsel, and ask each vendor how it records consent and opt-outs.

Can AI agents give patients medical advice?

The vendors in this guide that address it say no: Hippocratic AI states its agents do not diagnose or prescribe, Artera states its agents do not provide medical advice, diagnosis or treatment, and Zynix AI agents route symptom questions to the on-call clinician by rule. Ask each vendor to show its escalation rules.

Which patient engagement programs matter most for ACOs?

The programs that most affect quality and cost are annual wellness visits, preventive screenings and other care gaps, follow-up within days of a hospital discharge, chronic care check-ins and medication refills. Choose a platform that can run each of them and report what was completed.
