Disclosure: Zynix AI makes one of the platforms in this guide. Every description of another vendor comes from that vendor’s own website as of November 6, 2026, linked in the source notes. This is not a ranking: products change, so confirm current capabilities with each vendor.

Key takeaways

Care management software helps care teams find the patients who need attention, build care plans, assign tasks, reach patients and document the work, including Medicare programs such as CCM and TCM. For ACOs and physician groups, the main options are enterprise suites, EHR-native tools, CCM program platforms and follow-through layers that run outreach.

  • Enterprise value-based care suites (Innovaccer, Arcadia, Health Catalyst, Lightbeam, Persivia) combine care management with population health data and analytics.
  • Epic’s population health tools keep care management inside the EHR for organizations that run Epic.
  • CCM program platforms (ThoroughCare, ChronicCareIQ) focus on enrolling patients, timing calls and billing CCM, RPM, PCM and related codes.
  • Zynix AI is a follow-through layer: care plans with owners, plus voice and text agents that run outreach and scheduling within your rules, with clinicians making every clinical decision.

What is care management software?

Care management software is the workspace care managers, nurses and coordinators use to run programs for patients who need more than a visit: people with chronic conditions, recent discharges, rising risk or open care gaps. It usually covers patient identification and prioritization, care plans and assessments, task lists, outreach by phone and text, documentation and time tracking, and reporting.

For ACOs and physician groups in value-based contracts, care management is where population health insight turns into work. The Embedded Care Management Platform page describes how we approach it; this guide compares the main types of software and the vendors in each.

What should ACOs look for in care management software?

Look for software that starts from the right patients, keeps every step owned and turns the work into billable, reportable activity. Ask every vendor to show these on your own sample data:

  • Patient identification. Risk stratification and worklists built from claims, every practice’s EHR and hospital ADT notifications, with the reason each patient is on the list.
  • Care plans and tasks. Plans with goals, interventions, an owner for each task and escalation when a step is missed.
  • Outreach. Calls and texts in the patient’s language, booked visits and reminders, with consent and contact rules your team sets.
  • Medicare program support. Time tracking and documentation for CCM (99490, 99439, 99487), TCM (99495, 99496), PCM and APCM, so the work is billable.
  • Clinical boundaries. Clinicians own clinical judgment, and the TCM interactive contact is made by the billing practitioner or clinical staff under their direction.
  • Integration. A written integration plan for each EHR your practices run, and how results flow back to clinicians.
  • Security. The SOC 2 Type II audit report itself and a BAA you can review before any PHI is shared.

The platforms, one by one

Innovaccer Care Management

Innovaccer says its “Care Management Copilot uses advanced AI to automate documentation, generate care insights, and predict patient risk.” Its care management page lists ambient documentation of patient interactions, smart summaries and care plan suggestions, predictive risk analysis, and integration with EHRs, claims and other systems. In April 2025 Innovaccer launched copilots and agents for care management, including agents that start outreach by SMS and voice, send reminders and handle scheduling, and an ED Follow-up Agent for post-discharge coordination.

Arcadia Ascent Care

Arcadia says that, “built into Ascent Care, care management brings care planning, task management, patient outreach, and coordinated workflows together in a single experience.” Its page lists AI-powered stratification, intelligent worklists with recommended next steps, automated enrollment and task routing, and multi-modal outreach. Arcadia launched the Ascent platform in October 2026 with a care navigation agent that summarizes patient context, prioritizes patients and drives outreach, with governance that keeps humans in control of patient-facing decisions. For ACOs it highlights MSSP and ACO REACH workflows.

Lightbeam TCM, CCM & Rising Risk Management

Lightbeam, founded in 2012, offers integrated TCM, CCM and rising-risk management that combines AI-enabled risk prediction with automated patient engagement. Its pages list prioritization of at-risk patients using clinical, behavioral and SDOH factors, configurable alerts and patient rules, automated care workflows, and device-free remote monitoring by landline or basic phone across 30+ clinical programs. Lightbeam serves ACOs, MSOs, payers and provider organizations, and also offers advisory services for care management workflows.

Health Catalyst

Health Catalyst presents care management as care transformation within its population health and value-based care offering, to “support proactive, coordinated care that reduces avoidable utilization and improves patient outcomes.” It lists stratifying populations, targeted workflows and dashboards for care teams, automated discharge plans and follow-up reminders, alerts for missed follow-ups and caseload optimization. Its named care products include Care Flow and Lumeon care orchestration, which it acquired in 2024.

Epic Population Health (Healthy Planet)

Epic’s population health page describes care management across the continuum: “Assess patient needs, connect them to resources, and track their outcomes.” It lists care gap closure with labs, risk scores, paid claims and external data, outreach campaigns, a community resource directory, contract analytics and ACO quality reporting. Epic customers refer to the care coordination platform as Healthy Planet, and Epic’s social care coordination module is Compass Rose.

ThoroughCare

ThoroughCare, established in 2013, calls itself “an end-to-end solution for Chronic Care Management.” Its pages list care plans with SMART goals and evidence-based assessments, worklists that sort patients by care manager, status, billable time and risk, task assignment, SDOH assessments, automatic assignment of CCM CPT codes and secure texting. It supports programs from CCM, PCM and BHI to TCM, APCM and annual wellness visits, and lists ACOs, physician groups and health plans among the organizations it supports. Its TC Compass assistant adds AI-generated summaries and care plan goals.

ChronicCareIQ

ChronicCareIQ, founded in 2014, says it “supports CCM, RPM, RTM, PCM, BHI/CoCM, TCM, APCM, and CPM in one platform.” Its modules cover risk dashboards (RiskIQ), patient tracking (PatientIQ), billing-code calculation and audit-ready documentation (BillingIQ) and automatic capture of phone-based care time (CallerIQ). It positions itself as chronic care management software for high-volume Medicare practices and specialty groups.

Persivia CareSpace

Persivia describes CareSpace as a platform with an AI engine, care management workflows and quality reporting. Its population health page says each program “is powered by AI, automating eligibility, risk adjustment, risk stratification, care plans, clinical alerts, care gaps, and assessments,” with outreach by phone, SMS and telehealth. Its programs include AWV, CCM and TCM, and it lists health systems, ACOs, payers and risk-bearing provider groups among its users.

Zynix AI

Zynix AI is the AI operating layer for value-based healthcare, built for the step after a patient is flagged. Analytics tells you who. Zynix makes sure something happens. The Embedded Care Management Platform runs care plans for post-discharge follow-up, wellness visits and gap closure, with tasks, owners, deadlines and escalation by rule. AI Patient Engagement adds voice and text agents for outreach, scheduling, reminders and after-hours calls, working within the scripts and hours your team sets.

Agents never diagnose, triage symptoms or recommend treatment: they route symptom questions to your on-call clinician by rule, and licensed clinical staff make the TCM interactive contact. As of September 2026, Zynix has onboarded 1M+ value-based care patients and connects to 30+ EHR systems across 300+ connected instances. For one ACO’s setup, see the Palm Beach ACO case study.

How to choose: match the type to your gap

Pick the type first, then compare vendors within it.

  • You need one data and analytics foundation as well as care management: compare the enterprise suites (Innovaccer, Arcadia, Health Catalyst, Lightbeam, Persivia).
  • Your practices all run Epic: start with Epic’s population health and care management tools.
  • You run CCM, RPM or PCM in-house and billing is the bottleneck: look at CCM program platforms such as ThoroughCare and ChronicCareIQ.
  • Your lists are good but patients aren’t being reached: add a follow-through layer such as Zynix AI next to the system you already have, with one owner per worklist.

Whatever you shortlist, give every vendor the same demo script and ask for a dated plan built on your own data feeds before comparing prices.

Frequently asked questions

What is the difference between care management and care coordination software?

The terms overlap and vendors use both. Care management usually means running programs for individual patients: care plans, assessments, tasks, outreach and billable time for services such as CCM and TCM. Care coordination stresses handoffs between providers, settings and community services. Most platforms in this guide do both; ask each vendor to demo the specific workflow you need.

Does care management software handle CCM and TCM billing?

Many platforms track time and documentation for Medicare care management codes, such as CCM (99490, 99439, 99487) and TCM (99495, 99496). CCM program platforms such as ThoroughCare and ChronicCareIQ put billing at the center. Check that the software records consent, initiating visits, time by staff type and the TCM interactive contact within two business days of discharge.

Can AI agents do care management outreach?

AI voice and text agents can handle routine outreach: post-discharge check-in calls, wellness visit scheduling, reminders and gap-closure messages, within scripts and hours your team approves. They should not make clinical decisions. Ask how symptom questions are routed to a clinician, how consent and opt-outs are handled and how every conversation is recorded for review.

Can an ACO use more than one care management tool?

Yes. Many ACOs keep an enterprise or EHR-based platform for analytics and documentation and add a separate tool for outreach or a specific program. The rule is one owner per worklist: decide which system owns each program or gap so a patient is not contacted twice, and send outcomes back so every team sees the same status.

What should a care management software demo include?

Use your own de-identified sample: how a patient lands on a worklist and why, building a care plan with owners and deadlines, a completed outreach attempt, documentation and time capture for a CCM or TCM service, and the report a medical director would review. Give every vendor the same script so the answers can be compared.
