Disclosure: Zynix AI makes one of the platforms in this guide. Every description of another vendor comes from that vendor’s own website as of November 6, 2026, linked in the source notes. This is not a ranking: products change, so confirm current capabilities with each vendor.

Key takeaways

Population health management software brings claims, EHR and other data together so a value-based care organization can see who is attributed to it, who is at risk, which care gaps are open and how each contract is performing. Innovaccer, Arcadia, Health Catalyst, Lightbeam, Epic, Oracle Health, Persivia and Zynix AI all sell platforms for this work, with different emphases.

  • Start with data: list every source you need (claims, each practice’s EHR, hospital ADT notifications, labs, pharmacy) and ask each vendor which ones it ingests today, for organizations like yours.
  • Most platforms cover risk stratification, care gaps, quality measures and contract performance; they differ in who does the follow-up work and where it happens.
  • Enterprise suites (Innovaccer, Arcadia, Health Catalyst, Oracle Health, Epic) suit organizations that want one data and analytics foundation across many programs.
  • Zynix AI focuses on follow-through: care plans and voice and text agents that turn a risk list into completed calls, visits and care plan tasks, with clinicians making every clinical decision.

What is population health management software?

Population health management (PHM) software is the data and workflow layer an ACO, health system, MSO or health plan uses to manage the health and cost of a defined group of patients. It combines claims and clinical data into one record per patient, assigns patients to programs and contracts, flags risk and open care gaps, and tracks quality and financial performance against each value-based contract.

The category overlaps with care management software, which runs the care plans and tasks, and with patient outreach tools, which make the calls and send the texts. Some vendors sell all three in one suite; others focus on one layer and connect to the rest.

What should you look for in a population health platform?

Judge a platform on the data it can use, the questions it answers and how its answers become work your team actually finishes. These are the areas worth a written answer from every vendor:

  • Data sources. Claims (including Medicare CCLF for ACOs), each practice’s EHR, hospital ADT feeds, labs, pharmacy and social needs data, and how often each refreshes.
  • Attribution. Whether the platform reproduces your program’s attribution logic and shows why a patient is attributed.
  • Risk stratification. Which risks it predicts, which inputs drive each flag, and whether a clinician can see and check the drivers.
  • Quality and care gaps. Which measure sets it supports (for example ACO quality measures and HEDIS) and how gaps reach the people who close them.
  • Contract performance. Cost, utilization and quality tracking per contract, so leaders see where effort pays off.
  • Follow-through. Who makes the call, books the visit and updates the care plan once a patient is flagged, and how that work is tracked.
  • Security. The SOC 2 Type II audit report itself and a BAA you can review before any PHI is shared.

The platforms, one by one

Innovaccer Atlas

Innovaccer positions Atlas as its population health product, one that “connects data, intelligence, and delivery for every patient and population.” Its Population Health AI, part of Atlas, “reads every value-based contract you hold, assigns a value to every action, and shows your team where effort is worth the most.” Atlas runs on Innovaccer Gravity, which Innovaccer says unifies EHR, claims, financial and operational data. Innovaccer’s value-based care pages list risk and care gap identification, quality reporting, AI-assisted care management and contract performance tools.

Arcadia Ascent

Arcadia describes the Ascent Healthcare Performance Platform as “AI-native by design”, combining a data foundation with Ascent Connect “to unify clinical, claims, and operational systems.” Its product page lists quality measurement, risk adjustment, patient prioritization, care coordination, self-service analytics, and monitoring of contracts, attribution and payment performance. Arcadia says Ascent uses “governed, purpose-built agents” that keep humans in the loop, and it lists providers, payers, life sciences, ACOs and government among the organizations it serves.

Health Catalyst

Health Catalyst’s Population Health & Value-Based Care offering aims to “unify your data, understand your populations, and deliver targeted interventions that improve care and outcomes at scale.” Its population health management page lists combining clinical, claims and SDOH data, predicting risk across clinical, utilization and cost domains, automating CMS and HEDIS metrics, reassessing attribution models, prioritizing patients for outreach and role-based dashboards. Its Value-Based Care Performance Suite is built on reconciled clinical and claims data for performance across contracts, populations and provider networks.

Lightbeam Health Solutions

Lightbeam, founded in 2012, describes itself as a population health platform that helps providers and payers “manage risk, align incentives, and improve outcomes.” For ACOs it lists a view of the attributed population built from EHR, claims, ADT, HIE and SDOH data, quality tracking across MSSP, REACH, MIPS and payer programs, risk adjustment and attribution, and dashboards at the ACO, TIN, CCN and provider level. It also offers TCM, CCM and rising-risk care management, and in March 2026 it announced Microsoft Copilot-based Analytics Copilot and Cohort Copilot features.

Epic Population Health (Healthy Planet)

Epic’s population health software helps organizations “monitor a patient’s care and take action with precise engagement” wherever the patient receives care. Its page lists care gap closure using labs, risk scores, paid claims and external systems, contract analytics, point-of-care prompts for conditions and risk capture, social care resources, outreach campaigns and ACO quality reporting to CMS. Epic customers refer to the care coordination platform as Healthy Planet.

Oracle Health

Oracle Health groups its population health products as population and performance intelligence: Data Intelligence (a self-service enterprise data warehouse and analytics solution), Quality Management, Care Coordination Intelligence and Determinants of Health. Oracle says its AI-assisted tools help identify social determinants of health, care gaps, hierarchical conditions and readmission risks while working in any EHR, and that Care Coordination Intelligence helps summarize longitudinal records and enhance patient outreach.

Persivia CareSpace

Persivia describes CareSpace as “a digital health operating platform that unifies data from EHRs, claims, labs, SDOH sources, remote monitoring, and more into a continuously updated longitudinal patient record,” with its Soliton AI engine, care management workflows and quality reporting on top. It lists health systems, ACOs, payers and risk-bearing provider groups among its users.

Zynix AI

Zynix AI is the AI operating layer for value-based healthcare, built for teams that already know who needs attention and need the follow-up done. Analytics tells you who. Zynix makes sure something happens. Population Intelligence brings claims, EHR, ADT, lab and pharmacy data into one patient record, and Predictive Risk predicts readmission, mortality, disease progression and utilization, with the drivers shown. The Embedded Care Management Platform turns each flag into a care plan with an owner, and AI Patient Engagement runs voice and text outreach, scheduling and reminders within the rules and hours your team sets.

Agents never diagnose, triage symptoms or recommend treatment; they route symptom questions to your on-call clinician by rule. As of September 2026, Zynix has onboarded 1M+ value-based care patients, serves customer organizations in 30 states and connects to 30+ EHR systems across 300+ connected instances.

Where do Navina and other tools fit?

Some tools that come up in the same searches are not population health platforms. Navina, for example, describes itself as “the clinician-first AI copilot for value-based success”, focused on risk adjustment, quality and care gaps at the point of care. Tools like this sit beside a PHM platform rather than replacing it.

Independent research firms also rate this category. KLAS Research named Azara Healthcare the 2026 Best in KLAS for Population Health Management. Vendor-written “top 10” lists are common too; read them knowing who wrote them, and ask every vendor for references from organizations like yours.

How to choose: match the platform to your gap

Choose by the problem you need solved first, not by the length of the feature list.

  • No trusted data foundation yet? Start with an enterprise platform that can ingest every source you have, such as Innovaccer, Arcadia, Health Catalyst or Oracle Health.
  • Already on Epic everywhere? Epic’s population health tools keep the work inside the same EHR.
  • An ACO or MSO focused on MSSP or REACH reporting? Compare Lightbeam, Persivia and the enterprise suites on attribution, quality and dashboards for your exact program.
  • Lists are fine, but follow-up isn’t happening? A follow-through layer such as Zynix AI can run outreach, scheduling and care plan tasks next to the analytics you already have, as long as each worklist has one owner.

Whichever you shortlist, ask for a dated plan built on your own data feeds and measure results on your own population. For a side-by-side with one of the suites above, see Zynix AI compared with Innovaccer.

Frequently asked questions

What is the difference between population health management and care management software?

Population health management software works across the whole population: it combines data, attributes patients to contracts, stratifies risk, finds care gaps and tracks quality and cost. Care management software runs the work for individual patients once they are flagged: care plans, tasks, outreach and documentation. Many vendors sell both, but it helps to evaluate each layer on its own terms.

What data does a population health platform need?

At minimum, claims (for ACOs, Medicare CCLF files) and clinical data from every participating practice’s EHR. Hospital ADT notifications, lab results, pharmacy data and social needs data make risk flags and care gaps more timely and complete. Ask each vendor which sources it ingests today for organizations like yours, how often each refreshes and who maintains the interfaces.

Which population health platforms do ACOs use?

ACOs use enterprise platforms such as Innovaccer, Arcadia, Health Catalyst and Oracle Health, EHR-based tools such as Epic’s population health software, and ACO-focused platforms such as Lightbeam and Persivia. Many add a follow-through layer for outreach and care plans. The right mix depends on how many EHRs your practices run and which program you report to.

Can an organization use two population health tools at once?

Yes, and many do: for example an enterprise analytics platform plus a separate tool for outreach and care plans. The rule that matters is one owner per worklist. Decide which system owns each gap or program so a patient is not contacted twice about the same thing, and feed results back so both systems show the same status.

How long does it take to implement population health software?

It depends mostly on data. Each EHR interface, claims feed and ADT connection has to be set up and validated, and interfaces differ by EHR version and configuration. Ask every vendor for a written integration plan per practice and a dated plan to the first live workflow, built on your own feeds, before you compare prices.
