Zynix is an approved CMS ACCESS participant. Any ACO or practice can take part through us: your clinicians refer eligible Original Medicare patients, stay involved in their care, and earn the ACCESS co-management fee. No ACCESS application of your own.

How it works
- ReferYour clinicians refer eligible patients to Zynix’s ACCESS program. Referring clinicians don’t enroll in ACCESS.
- We run the programZynix handles enrollment outreach, technology-supported care, clinician escalation and outcome reporting.
- Co-manage and billYour clinicians review our updates, coordinate care, and bill the ACCESS co-management service for that documented work.
The co-management fee
- About $30 per co-management service
- Up to 3 per patient, per track, each year
- About $10 more for first-time onboarding

About ACCESS
CMS’s ACCESS Model began July 5, 2026 and runs ten years. It pays for technology-supported chronic care, with full payment tied to measurable outcomes. Its tracks cover eCKM, CKM, MSK and behavioral health; ask us which we run today.
Zynix integrates with 30+ EMRs, including Epic, Cerner (Oracle Health), athenahealth and eClinicalWorks.
